Important updates on topics affecting Select Health AllianceFocusing on Population Health Management
This month’s SHA Newsbyte focuses on the concept of the Population Health Management — the active coordination of care across a defined population to prevent and manage disease, with the aim to improve clinical outcomes and lower the associated cost of care delivery.
Baptist Memorial Health Care has agreed to let SHA manage its employees, and I am asking for volunteers to help build this platform and work on this initiative.
Many of you have already started using patient portals to enhance communication. Virtual management meets many patient demands for convenient ways to engage in long-term physician relationships, while at the same time freeing up office practice staff for more critical in-office needs. It’s a big step toward SHA’s goal of keeping physician reimbursement at a reasonable level in the face of marketplace changes.
