Focusing on Patient Centered Medical Home

This month’s SHA Newsbyte focuses on the concept of the “Patient Centered Medical Home,” sometimes called “Advanced Medical Home.” Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model that delivers the core functions of primary care and promises to transform how primary care is organized and delivered.

The five core attributes of a medical home include:

  1. Comprehensive care
  2. Patient-centered care
  3. Coordinated care
  4. Accessibility
  5. Quality and safety

Our hope is that at some point you strive to reach all of these, but right now SHA’s focus is coordinated care.
