Goals for Individual Patients

Ideally, the patient should strive for:

  • HgbA1C <7.0%
  • Blood pressure <130/85
  • Males 40 or older on statin medication
  • Females 50 or older on statin medication

Goals for Aggregate Community of Diabetics

Clinical Measure Measure Description Goal

HbA1C < 9.0%

Percentage of patients 18 to 75  years of age with diabetes who had HbA1c < 9.0% during  measurement period ◊

≥ 85 % of patients◊

Blood Pressure Control ≤ 140/90

Percentage of patients 18 to 75 years of age with diabetes who had a diagnosis of hypertension and whose blood pressure was controlled (≤ 140/90 mmHg) during measurement period ◊

≥ 65 % of patients◊

Diabetes: Medical Attention for Nephropathy

The percentage of patients 18 to 75 years of age with diabetes who had a nephropathy assessment (i.e. micro albuminuria test) or medical attention for nephropathies during the measurement period ◊ Medical attention may include:

  • Any visit to a nephrologist
  • Patient receiving dialysis, receiving
  • treatment for ESRD, CRF, ARF, or renal insufficiency;
  • Receiving ACE inhibitor or ARB therapy

≥ 85% of patients◊

Diabetes: Eye Exam

Percentage of patients 18 to 75 years of  age with a diagnosis of diabetes (Type I & II) who had a retinal or dilated eye exam in the measurement period or a negative retinal or dilated eye exam (negative for retinopathy) in the year prior to the  measurement period ◊

≥ 60% of patients◊

Preventive Care and Screening: Tobacco Use; Screening & Cessation Intervention

Percentage of patients with Diabetes aged 18 years and older who were screened for tobacco use one or more times within 24 months AND who received cessation  counseling intervention if identified as a tobacco user Δ

≥ 85% of patients◊

Foot Examination

Percentage of patients aged 18 to 75  years of age with diabetes who had a foot exam during the measurement period ◊

≥ 80% of patients◊
